Are these repair costs adjusted for where I live?

The comparison table on this page holds labor and materials at the national baseline so the tools are comparable side by side. Open any calculator, set your state, and the same model rescales contractor labor and material dollars to local rates — across the 51 jurisdictions FixTally models, labor runs from 0.80× to 1.45× the national baseline (Hawaii and Alaska sit at the top; the deep South at the bottom), so the same repair can legitimately quote ~80% more in the priciest state than the cheapest. Do these estimates include permits, disposal and repair of whatever got opened up?

It varies by trade, and each tool states its inclusions. Foundation repair includes the structural engineer's report and permit by default; water damage and mold include containment and disposal of removed materials. What most tools exclude is cosmetic restoration after structural or mitigation work — drywall, paint and flooring that gets opened to reach the problem — because that is usually a separate contract.
